The User Guide provides detailed information on the steps involved in the assessment process. A summary is provided below.
Summary of Self Assessment Process:
Step 1: Create an NCID user name and password using the NCID New User Registration Page. Please follow the instructions on the NCID New User Registration Page to activate your NCID username and password. If you already have an NCID username and password, please go to step 2.
Step 2: Use the login button to enter your NCID user name and password to gain access to the assessment site.
Step 3: Complete your User Profile.
Step 4: Identify the facilities you will be registering by entering their NPIs and confirming the provided information. After you complete this step NC DPH will activate your assessments. You will be notified when your assessments have been activated.
Step 5: Login to the assessment site to complete infection control assessments for your identified facilities. You can save and come back as many times as needed to complete the assessment. Users must answer all required questions for the assessment to be considered complete. Use the Status tab on the assessment to track your progress.
